This series presents a new paradigm for better care of the chronically ill and its application in quality improvement, policy development and research. Emphasis is given to research evidence upon which the chronic care model is based and evaluations of its utility. Presentations describe and exemplify delivery system changes included in the model and quality improvement strategies by which various healthcare organizations have improved their care. The lectures in this series were taped at the 2004 Epidemiology, Biostatistics and Clinical Research Methods Summer Session co-sponsored by the Seattle VA Epidemiologic Research and Information Center (ERIC) and the University of Washington.
